Softball tryout evaluation form

One sheet per player, built the way good softball tryouts run: a 1–5 rating at each skill station, a box for every measured rep, and room to say why. Print one per kid, clip them to a board, and you’ll have the same notes on every player when it’s time to pick the team.

No sign-up, no email. Print as many copies as you need.

Score against this group: 1 = well below, 2 = below, 3 = at level, 4 = above, 5 = well above. Leave a station blank if you didn’t see it.

What’s on it

  • Player details: name, tryout number, age group, positions, bats/throws and date.
  • 7 rated stations: hitting (live or front toss), infield defense, outfield defense, pitching mechanics, catcher receiving and blocking, athleticism, coachability and baseball iq.
  • Measured stations with three reps and a best: exit velocity, arm velocity, pitching velocity, pitching strikes (of 10), catcher pop time, home to first.
  • Notes, the evaluator’s name, an overall score and a yes / maybe / no.

Rather do it on your phone?

TryoutSheet is this sheet on a phone: tap a player’s number, tap a score, film the rep. It works at the field without signal, adds up the stations with your weights, ranks the group, and carries the players you pick into your roster.

How to use it

  1. Give every player a number at check-in and write it at the top of their sheet. Evaluators score numbers, not names, which keeps the scoring fair.
  2. Put one evaluator at each station and have them rate every player there, rather than one coach rating a few players everywhere. You get scores that compare.
  3. Rate against this group, not against the best player you’ve ever coached: 3 is “at the level of this tryout”. Leave a station blank if you didn’t see it.
  4. For measured stations, write every rep and circle the best one. The radar or stopwatch settles arguments the 1–5 can’t.
  5. Before you choose, add up the sheets the same way for everyone. Weight the stations that matter most for your team (hitting and defense usually count more than one 60-yard run).

Questions

What should a tryout evaluation form include?

The skills you’ll pick the team on, each with the same scale, plus the measured numbers (exit velocity, arm velocity, home to first, pop time) and a few lines of notes. Keep it to one page so evaluators actually fill it in.

Should I use a 1–5 or 1–10 scale?

A 1–5 scale is easier to use consistently across several evaluators. With 1–10, coaches drift: one coach’s 7 is another’s 9. With 1–5 and “3 = at level for this group” everyone scores the same way.

How many evaluators do I need?

One per station is ideal, at least two for hitting and pitching. Use people who don’t have a child trying out, and have them score by tryout number.

Can I score this on my phone instead?

Yes. TryoutSheet has the same stations as this form, works at the field without signal, adds the scores up with your weights and ranks the group for you. It’s free for one tryout of up to 25 players.
